MomijiHealthCareSociety is a non-profit organization providing services to senior citizens of primarily Japanese-Canadian descent, in an effort to maximize their independence, health, and quality of life. We are currently seeking a full-time ProgramWorker for our Client Intervention department.
Working Hours :
37.5 hours per week, between the hours of 9:
00am and 5:
00pm, Monday - Friday • Position is located Momiji Centre and some remote hubs in the community Hourly Rate :
$19.50 Purpose/Scope :
To provide support to the client intervention function of the organization and the Client Intervention Worker.
Major Responsibilities:
Develop, support, and deliver informational and educational programs e.g.
workshops, tax return clinic.
Attend specific external service providers’ meetings as delegated to by the Client Intervention Worker.
Conduct assessment and supportive interview with seniors and their family as delegated to by the Client Intervention Worker.
Input into and maintain client files.
Research and Identify service and information resources in the community relevant to the needs of seniors seeking residential and supportive care at Momiji.
Organize and update the information resources for tenants, seniors, caregivers.
Develop and update resources in support of client intervention practice.
General Responsibilities:
Maintain and update applicants’ files.
Collect, organize, and report on data/statistics as directed by the Client Intervention Worker.
Prepare, analyze, and report on statistical information in relation to client intervention services and assist with corrective action to ensure department goals are achieved.
Provide other administrative support to the Client Intervention Worker including taking minutes at the Admissions Committee and others.
Participate in other duties as assigned.
Minimum Knowledge :
Experience in working with the elderly, particularly frail and cognitively impaired ones.
Strong computer literacy Proficient in Microsoft Office and other software Strong organizational skills Excellent assessment, interviewing, and problem-solving skills.
One-on-one and group work experience.
Familiarity with relevant legislation and standards related to community services.
Knowledge of community resources.
Ability to write reports.
Excellent communication skills, written and verbal Japanese language skills are a definite asset Valid driver’s license and access to and use of a car will be considered an asset.
Required Credentials :
Diploma in Social Service Work, or a combination of education and work experience Must be registered in good standing under the Ontario College of Social Workers and Social Services Workers (OCSWSSW).
